Community Support Services
Helps Monmouth County adults diagnosed with a mental illness successfully integrate into the community. As each consumer has individual needs, the Community Support Services program works with consumers to create an individualized rehabilitation plan.
Family Crisis Intervention Unit
A statewide program available in Monmouth County through a Youth Services Commission grant, FCIU provides information, referral, and case management to youth ages 10-17 and families. FCIU stabilizes youth in crisis and prevents involvement with the juvenile justice system.
Projects for Assistance in Transition from Homelessness (PATH)
Provides linkages to help consumers develop stability and remediate homelessness. The program is available to Monmouth County adults diagnosed with a mental illness who are homeless (or at imminent risk) and are not receiving other case management services.
