Supportive Housing/Partners in Independance

Free supportive housing services and/or subsidized housing to adults diagnosed with serious mental illness and are at risk for homelessness. Services include but are not limited to clinical assessments, counseling, case management, and transportation. The goal of these programs is to promote and sustain independence in permanent housing, and to assist and support each resident so as to become fully integrated into the community in which they reside.

Traditional Supportive Housing (PII)

Referrals for this program may come from anyone in the community including self-referrals. This program is funded by the State of New Jersey Department of Human Services, Division of Mental Health and Addiction Services (DMHAS) and the Monmouth County Department of Human Services, Division of Mental Health and Addiction Services.

Enhanced Supportive Housing (PII2)

This program provides services to individuals who are being discharged from state psychiatric hospitals. Referrals for this program must be made by designated New Jersey Division of Mental Health and Addiction Services staff. This program is funded by the State of New Jersey Department of Human Services, Division of Mental Health and Addiction Services (DMHAS).

At-Risk Supportive Housing (PII+)

This program provides services to individuals who are at imminent risk of homelessness and/or hospitalization. Referrals for this program must be made by designated screening centers, emergency services, and designated community providers. This program is funded by the State of New Jersey Department of Human Services, Division of Mental Health and Addiction Services (DMHAS).
