Projects for Assistance in Transitioning from Homelessness (PATH)

Projects for Assistance in Transitioning from Homelessness (PATH) remediates homelessness (or imminent risk) for adults with mental illness who are not receiving case management and provides linkages to develop client stability.

Mental Wellness: By the Numbers
In 2019, PATH performed 108 consumer outreaches, admitted 101 individuals and families, with 159 served.

Eligibility
Our PATH case managers assist Monmouth County residents who are 18 years of age and older, are homeless (or at imminent risk), and have been diagnosed with a severe mental illness.

Consumers cannot currently be receiving case management services.
